Skilled Nursing Facility is searching for MDS Assessor in Queens, NY.
RESPONSIBILITIES: - Perform precise evaluations, including MDS and care plans, as directed
- Initiate and manage care plans and related activities as assigned
- Prepare and distribute monthly care plan schedules promptly
- Update and maintain care plans and evaluations in accordance with requirements
- Review and audit clinical records for accuracy and timely completion
- Report any recurring issues regarding non-compliance with documentation to the Director of Nursing
- Ensure the confidentiality of resident and facility information at all times
QUALIFICATIONS: - Prior MDS 3.0 experience is a MUST
- Active RN license in New York
- Previous experience in long-term care settings required
- Strong organizational skills and a professional, self-driven approach
- Proficiency in computer systems
- Exceptional communication abilities
- Friendly, collaborative attitude and a team-oriented mindset
