Sky View Rehabilitation and Health Care, a 192-bed Skilled Nursing Facility located in Croton-on-Hudson, NY has an opening for an opening for a
Part-Time Medical Records Clerk in its Nursing Department. Sky View serves as the bridge between hospital and home, spanning the gap from recovery to community reentry.
This position requires providing clerical support to the nursing units including but not limited to: current and overflow filing, ensuring charting is up-to-date, and coordinating the flow of paperwork:- Medical Records Clerk, Collect, assemble, make readily accessible, systematically organize, complete, and file resident medical charts and paperwork as required.
- Medical Records Clerk, Files all resident data upon receipt and approval of Nurse Manager/Designee. The medical record files are catalogued periodically and placed in an appropriate manner according to the alphabetical order or numeric system of filing. Documentation must be filed by year and mirroring the active medical record chart order.
- Medical Records Clerk, Ensures that incomplete records are communicated to the appropriate department and follows up in appropriate time frame. Notifies Administrator/DNS/Medical Director of out-of-compliance issues in a timely fashion to ensure completion and compliance.
- Medical Records Clerk, Ensures resident charts are in good working order: add new forms, label pages, reinforce documents, etc.
- Medical Records Clerk, Safeguards clinical record information against loss, destruction, or unauthorized use. Tracks, maintains and reports to the Corporate Compliance Officer any suspected unauthorized information taken from charts.
- 6. Routinely thins medical record chart as per facility policy. Reviews documents and identifies missing or inaccurate documents, notifies Nurse Managers.
- 7. Thins, files and maintains all overflow charts.
- 8. Stays up-to-date with all federal and state guidelines for medical records. Assists in developing procedures to ensure active and inactive medical records meet appropriate standards and guidelines.
- 9. Follows appropriate procedures for the maintenance and security of active and inactive medical records.
- 10. Ensures supplies and forms (incident reports, new admission/readmission, DNR, etc) are sufficient, and brings copies to floors as needed. Delivers paperwork timely (lab results, etc) to floors as received.
Requirements:
- Experience in a health care facility/setting (hospital or nursing home) preferred
- Must be reliable, organized and possess excellent communication skills
