A healthcare facility is seeking a Full-Time UAS Registered Nurse to provide care for our residents. This position involves educating patients and their families about prevention and promoting healthy habits. Key Responsibilities: - Conduct in-person comprehensive clinical assessments for current residents based on UAS guidelines, policies, and state regulations.
- Develop service plans and assess eligibility for continued program participation through UAS evaluations.
- Assess potential new members holistically (clinical, social, and environmental factors) to determine the necessary level of services for their safety and well-being within the community.
- Collect clinical data from various sources to address the patient's conditions, following established guidelines, policies, and clinical judgment.
- Communicate effectively with patients, families, providers, and relevant parties to ensure the accuracy of the comprehensive assessments.
- Demonstrate a deep understanding of the needs of vulnerable populations with chronic or complex bio-psychosocial conditions.
- Complete all assessments in alignment with regulatory standards and internal policies.
Qualifications: - Active Registered Nurse (RN) license required.
- Proficiency with UAS-NY preferred.
- Strong communication, organizational, and interpersonal skills.
- Ability to work autonomously and efficiently.
- Competency in computer usage and English language communication.
- Experience in MLTC/Managed Medicaid preferred.
