We are seeking a passionate Nurse Practitioner with suboxone waiver to join our team! The Nurse Practitioner will be providing comprehensive evaluations and treating the adult substance abusing population, mentally ill chemical abusers and mentally ill patients. The Nurse Practitioner will assess prospective patients for admission to detoxify and to rehabilitate to the facility, determine appropriate level of care and address any immediate ongoing health issues that can be assisted at the residential, rehabilitation, or detox level of care.
Essential Duties & Responsibilities:
- Conduct evaluations with relevant history, health assessment based on the patient’s signs & symptoms.
- Identify needs of the patient and make a differential diagnosis based on the clinical presentation, history and clinical findings.
- Conduct waived testing for urine, blood work and glucose monitoring.
- Develop a prioritized and mutually acceptable plan of care according to the written practice protocols in collaboration with the physician.
- Implement a treatment plan that is prioritized and mutually acceptable with appropriate diagnostic tests, pharmacological and non-pharmacological interventions, patient education, and with appropriate referrals as needed.
- Provide systematic follow-up by evaluating the effectiveness of various treatments provided, therapeutic processes used, their outcome and modifying treatments & management plans as necessary to achieve the optimum mental health goals.
- Provide educational materials & information needed to make well informed choices and decisions by patients about their health promotion, illness prevention and finally, illness treatment.
- Provide a safe environment for the patient, and promote patient advocacy at the individual, community, and global levels.
- Participate with other professional colleagues to provide comprehensive health care.
- Maintain accurate, legible, and confidential medical records.
Requirements Minimum Qualifications:
- Master's Degree
- Current/valid New York State Nurse Practitioner (NP) license
- Currently registered as an NP certification in good standing
- Certified as a nurse practitioner pursuant to 6910, with prescription writing privileges pursuant to § 6902.3b of the Education Law
- DEA license
- Hold all requirements indicated by the NY Department of Drug and Alcohol Program to perform history and physicals and prescribe medications
- Current basic life support for health providers (BLS)
- Five (5) years of related work experience
- ANCC credentialing and certification
