Discharge Planner
The Riverside Premier Rehabilitation and Healing Center is proud to announce that we are seeking a full-time Discharge Planner!
We are seeking a full-time nursing home discharge planner to provide, coordinate and facilitate discharge plans from nursing home setting to community, and/or other healthcare facilities. Good communications skills between families, patients, other healthcare providers and the discharge planner will be essential. Must be able to multi-task, efficiently arrange and execute discharge plans in a timely manner as well as be people-oriented and compassionate. Will need to collaborate with disciplinary team, patient and families to proactively develop and implement a safe and appropriate discharge plan. Discharge PlannerRequirements:
Maintain a current list of resources for referrals and refer to the appropriate inpatient, outpatient and community resources.
Maintain knowledge of Medicare, Medicaid and private insurance company coverage for referred products and services.
Communicate with admissions & social work directors regularly regarding discharges.
Identify patients for teaching, discharge, and extended care facility needs.
Participate in team meetings conducted for interdepartmental collaboration with the patient and their family as deemed necessary, this includes multidisciplinary meetings and Care Plan meetings.
Assist as needed with obtaining referrals, prior authorization for Home Health Care, DME, SNF, acute rehab and appointments.
Education and Experience:
Bachelor's degree in social work or closely related field required.
One year of experience in the social services field require. Applicants with experience in a health care setting will be given preference.
