POSITION SUMMARY:
The Wound Care Nurse is responsible for developing, coordinating, monitoring, and managing the hospitals wound care program. Monitors and works to improve clinical outcomes.
Qualifications:
Graduate of an accredited School of Nursing
Current state licensure as a Registered Nurse/Licensed Practical Nurse.
BLS certification
Current IV certification
Wound care certification (within 2 years)
Job Responsibilities:
1. Develops and implements wound care program to include patient care protocols, documentation tools and wound care formulary.
2. Collaborate with Chief Clinical Officer to establish standards of care, competencies, policies and procedures in quality, cost-efficient, and effective skin care wound management for all clinical staff, including: Registered Nurses, Licensed Practical Nurses, Physical Therapist, Nursing Assistants, Dietician and Medical Doctors.
3. Empowers all clinical staff to effectively assess wounds, recommend appropriate protocols, and advocates with physicians.
4. Educates staff on protocols that preserve integrity of the skin and promote wound healing.
5. Oversees the ulcer prevention and management program including tracking and reporting pressure ulcers. Acquisition and appropriate use of specialty beds and other pressure reduction devices, compression devices and wound vacuum systems.
6. Identifies patient’s needs and communicates that need to the patient’s physician.
7. Provides basic wound care training and orientation to all new nursing and therapy staff on hire; provides ongoing in-services and training relating to skin and wound care.
8. Works with vendors to maintain cost effective wound care formulary.
9. Participates in performance improvement activities.
10. Participates in marketing programs as needed.
11. Conducts on-going surveillance using Center of Disease Control (CDC) infection criteria, documentation, and investigation of hospital acquired infections through review of admission diagnoses, microbiology culture results, isolation orders, patient records, consultation requests etc.
12. Conducts environmental rounds in all inpatient areas monthly.
13. Participates in quality/performance improvement activities by assessing, monitoring, and measuring hospital acquired infections and evaluating outcomes on a continuous basis.
14. Chairs regularly scheduled Hospital Infection Prevention meetings.
15. Serves as a knowledgeable and available resource on infection control practices and policies to patients, families, staff, and hospital employees.
16. Maintains compliance with national standards and regulation bodies such as Joint Commission, CDC, OSHA, and DHSS.
