Position Summary: The Social Model Program Director will lead and manage the Adult Day Care program, ensuring the delivery of high-quality, person-centered services that align with the social model of care. This role involves fostering a supportive environment that promotes social engagement, well-being, and independence for participants.
Key Responsibilities:
- Develop, implement, and oversee programs that enhance social interaction and community engagement for participants.
- Monitor and evaluate program effectiveness, making data-driven adjustments as needed.
- Create and maintain policies and procedures that support the social model of care.
- Lead, train, and supervise program staff, promoting teamwork and professional growth.
- Conduct regular staff evaluations and provide training opportunities to enhance service delivery.
- Build and nurture relationships with community organizations, healthcare providers, and families to facilitate participant engagement.
- Represent the Adult Day Care program at community events and forums to raise awareness and support.
- Manage program budgets and resources to ensure financial sustainability and efficiency.
- Identify and pursue grants and funding opportunities to enhance program offerings.
- Collect, analyze, and report program data to assess impact and inform future planning.
- Advocate for participants' needs and preferences within the community and healthcare systems.
- Stay updated on best practices and emerging trends in adult day care and social models of care.
Qualifications:
- Bachelor's degree in a healthcare field (preferred).
- Experience in program management, in adult day care or related social services (preferred).
- Strong understanding of the social model of care and relevant regulations.
- Excellent communication, leadership, and interpersonal skills.
- Proven ability to work collaboratively with diverse populations and stakeholders.
- Proficient in data management and reporting tools.
