Full-Time Social Worker to join our team at our
Skilled Nursing Facility! Responsibilities: - Supporting residents in maintaining or enhancing their ability to meet daily physical, mental, and psychosocial needs.
- Assessing and addressing individual social and emotional requirements.
- Creating and managing personalized care plans.
- Collaborating in the planning of resident admissions, community re-entry, or transfers to different facilities.
- Participating in interdisciplinary care meetings to address the social and emotional needs of residents.
Qualifications: - Bachelor's or Master's Degree in Human Services, Psychology, or Social Work.
- Experience in Long Term Care or Subacute settings is a plus.
